GLP 1 and Strength Training: Why You Cannot Skip the Weights
If you have been anywhere near social media, a doctor's office, or a group chat with your girlfriends in the last two years, you have heard about GLP 1 medications. Ozempic, Wegovy, Zepbound, the list keeps growing. And I get it. For a lot of people these medications have been genuinely life changing. Appetite finally feels manageable, the scale is moving, and the constant noise around food quiets down.
But here is the part nobody is telling you at the pharmacy counter. Losing weight is not the same as getting healthy. And if you are not strength training while you are on one of these medications, you are very likely losing muscle right alongside the fat, sometimes even faster than the fat comes off.
What Is Actually Happening to Your Body
GLP 1 medications work by suppressing appetite and slowing digestion, which means you are eating in a calorie deficit, often a pretty significant one. Your body does not know the difference between "I am dieting on purpose" and "I am starving." When it senses a big drop in calories coming in, it will pull energy from wherever it can get it fastest, and muscle tissue is an easy target if you are not giving your body a reason to hold onto it.
Research on these medications has consistently shown that a meaningful chunk of the weight lost, in some studies close to a third or more, comes from lean muscle mass rather than fat. That is a problem. Muscle is not just about how you look. It is your metabolism, your bone density, your balance, your ability to carry your kids or your groceries or get off the floor without thinking about it when you are seventy. Losing it quietly while the number on the scale drops is not a win, it is a trade you did not agree to.
Strength Training Is the Part of the Plan That Protects You
This is where lifting comes in, and it is not optional if you want this to actually work long term. When you strength train consistently, you are sending your body a clear signal that the muscle you have is being used and needs to stay. Combined with adequate protein, that signal is often enough to shift where your weight loss is coming from, protecting lean mass while the fat continues to come off.
You do not need to become a powerlifter. You need a program built around the big, functional movements. Squats, hinges, presses, pulls, carries. Hydration matters more than people realize, especially with the GI side effects some people experience on these medications. Dehydration will tank your energy in the gym and make recovery harder across the board.
Sleep is not a luxury here. It is where muscle repair actually happens, and it plays a real role in hormone regulation, which matters even more when you are already changing your body's chemistry with medication.
And consistency beats intensity every single time. Two or three solid strength sessions a week, done consistently for months, will do more for your body composition and your long term health than an occasional all out effort followed by weeks off.

GLP 1 medications can be a genuinely useful tool. But a tool is only as good as what you build with it. If you use one of these medications without a strength training foundation underneath it, you may hit a lower number on the scale while ending up weaker, softer, and more metabolically fragile than when you started. That is not the goal, and it does not have to be your story.
Pair the medication with real strength work, prioritize your protein, sleep, and hydration, and you give yourself the best shot at coming out the other side stronger, not just smaller. That is the difference between losing weight and actually getting healthier.
